Applix Benefits Overview

This Benefits Summary is intended to provide a general summary of the benefits available to employees of Applix, Inc. The Company reserves the right to make changes to the benefits program at any time. Specific information about eligibility and coverage is found in official plan documents and human resources policies.

Eligibility

All regular full-time employees scheduled to work a minimum of 30 hours per week are eligible to participate in the Applix, Inc. Group Health, Dental, Flexible Spending Accounts, Short-Term Disability, Long-Term Disability, Life, and Accidental Death & Dismemberment programs. Refer to benefit descriptions for eligibility requirements for other benefits included in this overview.

Health Plan

Health Insurance Benefits are administered by Tufts Benefit Administrators. Employees in Massachusetts and some other locations have access to network benefits. Eligible employees and their eligible dependents become effective on the employee's first day of employment, provided the employee applies for benefits within the first 31 days of employment. Applix pays the full cost of employee health coverage and a major portion of the cost of dependent coverage. Opt-out benefit is also available.

Dental Plan

Dental Insurance Benefits are provided by Delta Dental to a maximum benefit of $1,000 per person per calendar year. Eligible children under age 19 are eligible for orthodontic benefits to a lifetime maximum of $1,000. Employees in Massachusetts have access to network benefits. Eligible employees and their eligible dependents become effective on the employee's first day of employment, provided the employee applies for benefits within the first 31 days of employment. Applix pays the full cost of employee dental coverage and a major portion of the cost of dependent coverage. Opt-out benefit is also available.

Prescription Drug Plan

Prescription Drug Benefits are provided by Express Scripts. All employees and covered dependents enrolled in a company-sponsored health plan are eligible to participate in the prescription drug program. Network pharmacies are available nationally.

Vision Care Benefit

Vision Care Benefits are available to all employees and covered dependents enrolled in a company-sponsored health plan to a maximum of $250 per 24 month period. Covered expenses include: eye examinations, contact lenses, glasses and fittings.

Flexible Spending Accounts

Flexible Spending Accounts allow you, if eligible to use pre-tax dollars for medical, dental, and/or dependent care reimbursement of otherwise after-tax expenses. Your tax base and Social Security are lowered, resulting in a tax savings and an increase in your net pay. The maximum for qualified unreimbursed medical expenses is $1,500 per calendar year. The maximum for qualified dependent care expenses is $5,000 per calendar year.

Basic Life and Accidental Death & Dismemberment Insurance

Basic Life and Accidental Death & Dismemberment (AD&D) Insurance is provided through UNUM. Coverage is equal to twice your base annual salary to a maximum of $250,000 each for basic life and AD∓D insurance. Beneficiaries are designated on forms available in Human Resources. Applix pays the full cost of basic life and AD&D coverage.

Supplemental Life Insurance

Supplemental Life Insurance is provided through UNUM. This allows you to purchase supplemental life insurance for you and/or your dependents on a voluntary basis to specified maximums.

Short-Term Disability Benefits

Short-Term Disability Benefits are provided through Applix for non-work-related injuries and illnesses. A physician's certification of disability is required in order to receive benefits. You are eligible for short-term disability benefits after 3 months of continuous employment. Applix will continue to pay 100% of your base salary for the first 30 calendar days and 60% of your base salary for the next 60 calendar days, after accrued sick time has been exhausted. Applix pays the full cost of short-term disability benefits.

Long-Term Disability Benefits

Long-Term Disability Benefits are provided through UNUM. You may apply for long-term disability benefits after you have collected short-term disability benefits for 90 calendar days. You are required to provide a physician's certification of disability in order to receive benefits. If approved for long-term disability benefits, you will receive 60% of your total monthly salary to a maximum of $7,000 per month. Applix pays the full cost of long-term disability benefits.

Holidays

Applix provides 10 paid holidays per calendar year. Part-time employees are eligible to be paid for holidays that fall on scheduled work days only.

Vacation Time

If you have less than 3 years of service, you will accrue vacation time at a rate of 1.00 day per month (12 days per year), completion of 3-5 years 1.25 days per month (15 days), completion of 6-12 years 1.67 days per month (20 days), and completion of 13 years 2.08 days per month (25 days). Employees will be eligible to carry over vacation time from one calendar year to the next, in an amount not to exceed their annual vacation allowance rate, which is based on one's years of service, starting in 2000. Part-time employees are eligible for pro-rated vacation time. Vacation time must be approved in advance by your manager.

Personal Days/ Paid Time Off

Off The definition of personal days is changing to paid Time Off days. The number of PTO days are 3 days/year. The first two PTO days are allotted at the beginning of each calendar year and the third day is allotted on July 1. Any new employee hired after 5/1 will be eligible for two PTO days. Any employee hired after 9/1 will be eligible for 1 PTO day in the current calendar year. These days cannot be carried over into the following year.

Sick Leave Policy

You will accrue sick time at a rate of 7 days per year. Sick time is available for temporary absences due to injury or illness. You may also use sick time to attend doctor's appointments with the approval of your manager. You may accrue sick time to a maximum of 20 days at which time accruals will cease. Effective 1/1/2000, the max. number of Sick Leave days that one can accrue will decrease to 12 days. Part-time employees are eligible for pro-rated sick time.

401(k) Retirement Savings Plan

Applix offers a company matched 401(k) Plan through Fidelity Investments. This plan allows you to contribute 1%-15% of your gross compensation to a retirement savings plan on a pre-tax basis. Applix will match up to 6% of your gross compensation to a maximum of 1/3 of the amount contributed. Company matching begins upon enrollment. You may choose to invest your contributions in a combination of 10 Fidelity funds. You must work 1,040 hours per calendar year in order to be eligible to participate in the 401(k) Plan. Enrollment and changes are on a quarterly basis.

Employee Stock Purchase Plan (ESPP)

Applix offers an Employee Stock Purchase Plan (ESPP) to allow you to purchase Applix stock at 85% of the fair market value of the stock at the lowest closing price on the first or last day of the plan period. You must work a minimum of 20 hours per week to be eligible to participate in this plan. The plan period runs from February 1 through July 31 and August 1 through January 31 of each year. You may deduct 1%-10% of your base annual salary during the plan period. At the end of the plan period, your deductions will be used to purchase stock at 85% of the selected closing price. You may withdraw at any time during the plan period. Enrollment and changes are at the beginning of each plan period.

Tuition Assistance Program

Applix encourages employees to further their education and will support the opportunity for you to build on your current knowledge and experience. After you have completed 3 months of continuous employment, you are eligible for reimbursement for approved tuition expenses to a maximum of $4,500 per calendar year for undergraduate level courses and $6,000 per calendar year for graduate level courses. Reimburseable expenses include tuition, registration fees, and required books. Expenses for approved courses are reimbursed at 100% for a grade of C or better for undergraduate level courses and 100% for a grade of B or better for graduate level courses. Employee Assistance Program

Applix provides life balance resources through ComPsych. By calling 1-800-272-7255, you have access to a variety of resources for counseling and education to help face numerous demands both at work and at home. This service is completely confidential.

Employee Referral Program

In the interest of attracting high quality employees and minimizing recruiting and advertising fees, Applix will pay an employee referral bonus of $3,000 to qualified employees. Bonuses are paid to employees after the referred employee has completed 3 months of continuous employment. Both you and the referred employee must be employed by Applix at the time payment is due.

Credit Union

Applix provides credit union membership through the Digital Credit Union.

Discounts

Applix offers discount memberships at BJ's Membership Club.
